What you’ll do: In collaboration with physicians, therapists, and care teams, you’ll assess functional status, design individualized treatment plans, and apply evidence-based interventions to improve strength, mobility, balance, and endurance. You’ll guide patients through gait training, manual therapy, therapeutic exercises, neurorehabilitation strategies, and pain management approaches. You’ll monitor progress using standardized outcome measures, adjust plans as needed, document clearly, and educate patients and families to support home programs. You’ll participate in care coordination, discharge planning, and quality improvement initiatives, contributing to a culture of learning and excellence.
