In this Long-Term Care setting, your primary responsibilities will center on delivering high-quality, person-centered care under the LPN scope of practice. You will monitor patient conditions, administer medications within your licensure, assist with activities of daily living, and support residents’ nutrition, hygiene, and mobility needs. You’ll partner closely with RNs, physicians, and an interdisciplinary team to develop and implement individualized care plans, track changes, document outcomes, and advocate for residents’ safety and comfort.
