As a LTC RN on this assignment, your core responsibilities will include comprehensive patient assessments, medication administration from MARs, monitoring vital signs, and promptly addressing changes in condition. You’ll contribute to the development, documentation, and revision of individualized care plans in collaboration with physicians, therapists, social workers, and family members. Expect to lead or co-lead nursing rounds, coordinate transitions of care, manage wound care and dressing changes, perform catheter care as indicated, and implement evidence-based practices for infection control and fall prevention. You’ll educate residents and caregivers about disease processes, medications, and safety strategies to promote independence and confidence. Please note that hours and pay rates are estimates and may vary; final compensation packages and guaranteed hours will be confirmed during the hiring process.
