As an LPN in Long-Term Care, you’ll collaborate with RNs and interdisciplinary teams to provide comprehensive daily care. Your responsibilities will include administering prescribed medications and treatments with accuracy and timeliness, monitoring vital signs, assessing changes in residents’ conditions, and updating care plans in collaboration with the nursing team. You’ll perform wound care, assist with activities of daily living, support fall prevention and mobility, and implement infection control and safety protocols. Documentation will be precise and timely, ensuring continuity of care for residents and transparent communication with families. You’ll serve as a patient advocate, offering reassurance during difficult days and celebrating improvements with residents and their loved ones. This role provides opportunities to broaden expertise in geriatric nursing, chronic disease management, palliative and end-of-life care, and care coordination, with potential pathways toward leadership roles such as charge nurse or preceptorship for new staff.
