Role Specifics and Benefits: In this Long-Term Care LPN role, you’ll deliver high‑quality, resident‑centered care under the guidance of experienced RNs and physicians. Core responsibilities include administering medications and treatments with accuracy, monitoring vital signs and changes in condition, assisting with activities of daily living, wound care and dressing changes, catheter and ostomy care when indicated, and contributing to every resident’s individualized care plan. You’ll document assessments, updates, and outcomes clearly, communicating promptly with the broader health care team to adjust care as needed. You’ll also engage in resident education, foster mobility and safety, assist with discharge planning, and participate in family conferences to address questions and concerns.
