In this role you will deliver comprehensive, patient-centered care to residents in a long-term care setting. Key responsibilities include assessing resident needs, administering medications, monitoring vitals, managing wound care, and assisting with activities of daily living to sustain independence and dignity. You’ll contribute to care planning, document meticulously, and collaborate with registered nurses, therapists, and families to align care with each resident’s goals. You’ll implement fall prevention strategies, coordinate with interdisciplinary teams, and respond quickly and compassionately to changes in condition.
