In this role, you’ll lead comprehensive physical therapy interventions designed to restore function and improve quality of life for patients across ages and diagnoses. Key responsibilities include performing initial assessments and ongoing re‑evaluations, developing individualized treatment plans aligned with best practice guidelines, and delivering therapeutic modalities that blend manual techniques, therapeutic exercise, neuromuscular re‑education, gait training, balance and proprioception work, and adaptive equipment recommendations. You’ll document progress meticulously in electronic medical records, communicate clearly with physicians, nurses, speech therapists, case managers, and family members, and participate in discharge planning to ensure safe transitions home or to next care settings.
