In this Hospice RN role, you’ll be at the heart of patient-centered care. Key responsibilities include comprehensive symptom assessment and management, pain control, spiritual and emotional support for patients and families, and the development and ongoing refinement of individualized care plans. You’ll coordinate with physicians, social workers, chaplains, and other members of the interdisciplinary team to ensure seamless care transitions and holistic comfort. Documentation will be precise and timely, with a focus on quality metrics, compliance with regulatory standards, and continuous improvement. You’ll lead patient and family education on medication use, caregiving techniques, and at-home safety, while anticipating and mitigating potential crises.
