You’ll partner with physicians, case managers, pharmacists, and rehabilitation teams to develop and implement individualized care plans, aiming for optimal pain control, infection prevention, and functional recuperation. You’ll educate patients and families about diagnoses, treatment goals, medication safety, and post-discharge expectations, empowering them to participate actively in recovery.
