Your responsibilities include comprehensive medication management, therapeutic interchanges, drug utilization review, safe dispensing practices, and direct patient counseling at discharge and in ambulatory clinics. You’ll collaborate with physicians and care teams to optimize regimens, identify medication‑related problems, monitor adverse events, and contribute to pharmacovigilance initiatives.
