In this role, responsibilities include leading medication administration and safety, accurate documentation, monitoring for acute changes, performing wound care and preventive skin management, assisting with activities of daily living, and coordinating multidisciplinary care plans. You’ll facilitate ongoing patient education for residents and families, participate in discharge planning when appropriate, and contribute to quality improvement initiatives within the LTC setting.
