Role Specifics and Benefits:
- Core responsibilities include comprehensive resident assessments, development and execution of individualized care plans, medication administration, wound and infection control, routine IV/derivative therapies as appropriate, and monitoring of chronic conditions. You’ll lead daily rounds with CNAs, coordinate with therapy teams, and communicate clearly with families about goals and progress.
- You’ll contribute to safety and quality improvement through evidence-based practice, reduce unnecessary hospital transfers, manage fall risk, delirium prevention, and palliative care needs as indicated.
