- Role responsibilities: Provide direct patient care to residents in long-term care, including assistance with activities of daily living, routine monitoring, medication administration under physician orders, and timely response to evolving clinical needs. Monitor resident status, identify changes, document accurately, and escalate concerns to the supervising RN to ensure safety and continuity of care. Participate in care planning, patient and family education, and collaboration with the interdisciplinary team to optimize comfort, function, and quality of life. Assist with wound care, mobility support, fall prevention, infection control, and safety protocols, while upholding dignity and autonomy for each resident.
