In this role, your key responsibilities center on delivering high-quality, person-centered care. You’ll perform direct patient care under the supervision of an RN, administer medications, monitor vital signs, assess changes in condition, and participate in ongoing care planning to promote comfort, safety, and independence for residents with chronic illnesses, dementia, post-acute needs, and end-of-life considerations. You’ll support activities of daily living, assist with mobility, wound care, and symptom management, and document all interventions clearly in the electronic health record. You’ll collaborate with physicians, therapists, social workers, and family members to ensure seamless transitions and consistent communication. Your leadership will extend to supporting CNAs, coaching new staff, and contributing to a supportive unit culture that prioritizes safety, respect, and continuous learning. The role also offers clear pathways for professional growth within long-term care, including opportunities to deepen expertise in geriatric assessment, palliative care, infection control, and best-practice LTC protocols, with mentorship and ongoing education that recognize your aspirations.
