Key responsibilities include comprehensive assessment of residents, medication administration under state and facility protocols, wound care, ADLs assistance, mobility support, and fall prevention. You’ll collaborate with RNs, physicians, and interdisciplinary teams to develop and implement personalized care plans, document progress, and advocate for residents’ choices and safety. You’ll mentor junior staff, share best practices, and pursue professional growth in the long-term care specialty—whether expanding oversight, pursuing certifications, or taking on care coordination roles.
