In this LPN long-term care position, you will perform direct resident care, monitor health status, administer medications, assist with activities of daily living, participate in care planning, document thoroughly, and respond to changes with prompt assessment. You’ll collaborate with RNs, physicians, therapists, and social workers to coordinate treatments, prevent complications, and support quality of life for residents with chronic conditions, post-acute needs, or end-of-life support.
