Role specifics and benefits are designed to support your growth and well-being. In this position, you’ll conduct comprehensive patient evaluations to identify functional limitations and develop individualized treatment plans grounded in evidence-based practice. Documentation and communication with the interdisciplinary team are essential, ensuring seamless care transitions across settings. You’ll educate patients and families on home exercise programs, safety strategies, fall prevention, and lifestyle modifications to sustain gains between visits.
