In this role you will deliver essential long‑term care nursing responsibilities with intention and precision. Key duties include assessing patient needs, administering medications and treatments per physician orders, and monitoring vital signs and evolving care plans under RN supervision. You’ll perform wound care, diabetic foot care, and routine hygienic care; manage catheter and ostomy care as appropriate; and ensure safety through fall prevention and proper mobility assistance. Documentation in electronic health records, accurate reporting to the care team, and timely communication with families and primary physicians are core tasks. Collaboration is at the heart of the role: you’ll participate in round discussions, contribute to individualized care plans, and support transitional care when residents move between units or return home.
