Role Specifics and Benefits: As the Home Health Physical Therapist, you’ll perform comprehensive home-based evaluations, develop and implement evidence-based plans of care, and monitor progress toward functional goals. Your responsibilities include PT assessment, goal setting, gait and balance training, strengthening programs, wound management as appropriate, edema control, fall prevention strategies, ADL/IADL training, caregiver education, and safe transfers within the home.
