In this role you’ll:
- Provide compassionate, patient-centered care to residents, including daily living activities, monitoring of vital signs, timely administration of medications, and meticulous documentation in electronic health records.
- Collaborate with physicians, therapists, social workers, and family members to implement individualized care plans that emphasize safety, rehabilitation, and quality of life.
- Support discharge planning, prevention of complications, falls prevention, infection control, and mobility programs to maximize independence.
- Demonstrate clinical proficiency in wound care, basic IV management where applicable, and the management of chronic conditions common in long-term care populations.
