In this role, you’ll be at the forefront of patient-centered care coordination. Core responsibilities include assessing patient needs, developing and monitoring comprehensive care plans, coordinating services across inpatient, outpatient, home health, and community resources, and guiding smooth transitions from hospital to home or alternate care settings. You’ll partner with physicians, case management colleagues, social workers, and ancillary teams to manage risk, ensure payer compliance, and optimize outcomes. Your expertise will drive discharge planning, post-acute follow-up, and the removal of barriers to care—whether that means arranging transportation, securing durable medical equipment, or coordinating multidisciplinary rounds. You’ll educate patients and families, advocate for culturally competent care, and use data to shape improvements in pathways and policies. The assignment is scheduled with weekly pay in the range of $3,585 to $3,759, with guaranteed hours of 40 per week, and a start date of March 23, 2026. The duration is measured in weeks, with flexible extension opportunities to match patient needs and organizational requirements.
