Your core responsibilities will include comprehensive patient assessments to identify clinical needs and social determinants of health; coordinating with inpatient teams, discharge planners, primary care providers, and community resources to create efficient, safe transition plans; conducting utilization review to ensure appropriate length of stay and resource use; developing individualized care plans that align with payer requirements, evidence-based guidelines, and patient goals; facilitating communication across departments to prevent fragmentation of care; educating patients and families about chronic disease management, medication reconciliation, follow-up appointments, and red flags that require escalation; and meticulously documenting assessments, plans, and outcomes in electronic health records to support quality improvement and data-driven decision-making.
