In this role, you’ll evaluate physical and cognitive function, design and implement individualized treatment plans, and employ evidence-based interventions to maximize functional outcomes. Core responsibilities include comprehensive assessments, goal setting, and progress documentation; collaboration with therapists, nurses, physicians, and social workers to coordinate care across inpatient, outpatient, rehab, and home-care settings; modification of activities for safety, balance, motor planning, and adaptive equipment; and discharge planning with family or caregivers to ensure sustained gains. You’ll monitor progress, adjust plans as needed, and advocate for patient access to resources that support long-term independence.
