Role specifics and benefits: In this case management RN role, you’ll assess patient needs, create and monitor individualized care plans, coordinate services across acute, post-acute, and community settings, and lead discharge planning that aligns with payer guidelines and patient goals. You’ll collaborate with physicians, nurses, social workers, and home health providers to streamline referrals, coordinate resources, and remove barriers to recovery. Our compensation package reflects the value of your expertise, with weekly pay in the range of $2,209 to $2,331, and a competitive bonus structure.
