In this Hospice RN role, you’ll provide expert symptom management, comfort care, and goal-concordant planning for patients nearing end of life. Key responsibilities include comprehensive assessments, development and revision of individualized care plans, wound and symptom management, effective communication with patients, families, and the healthcare team, and ongoing education that empowers families to participate in care decisions. You will collaborate with physicians, social workers, chaplains, and case managers to ensure seamless transitions and consistent, compassionate support.
