As a Home Health RN, you’ll assess, plan, implement, and evaluate nursing care in patients’ homes, coordinating with physicians, therapists, and family caregivers. Core duties include medication oversight, wound care, IV therapy support, pain and symptom management, patient education, safety assessments, and fall prevention strategies. You’ll document findings, adjust plans of care with clinical judgment, and advocate for patients’ preferences and dignity.
