In this Long-Term Care LPN role, you will perform critical nursing functions under the supervision of a Registered Nurse and the facility medical director. Your responsibilities will include conducting comprehensive initial and ongoing resident assessments, monitoring vital signs, administering medications and injections per physician orders, managing MAR documentation, wound care, catheter care, and bowel and bladder management, and assisting with activities of daily living, mobility, and rehabilitation activities. You’ll contribute to individualized care plans, participate in interdisciplinary team rounds, identify changes in condition, and implement timely interventions to prevent complications such as falls or infections. You will champion infection control practices, adhere to regulatory standards, and maintain accurate electronic health records.
