- Conduct comprehensive PT evaluations and develop individualized, evidence-based treatment plans that align with patient goals and clinical guidelines.
- Provide expert therapeutic interventions, including therapeutic exercise, gait training, manual therapy, neuromuscular re-education, balance training, and pain management, adapting techniques to each setting.
- Monitor progress with objective measures, modify plans as needed, and communicate outcomes to patients, families, and the care team.
- Maintain meticulous documentation in multiple EHR systems, ensure regulatory compliance, and uphold patient safety and infection control standards.
- Educate patients and caregivers on home exercise programs, conditioning strategies, fall prevention, and community resources to support long-term outcomes.
- Collaborate with physicians, nurses, therapists, and social work to coordinate care, optimize discharge planning, and advocate for patient needs.
