In this Long-Term Care LPN role, you’ll own a core set of responsibilities that are essential to high-quality resident care. You will administer medications, monitor vitals, and document changes in patient conditions with accuracy and timeliness. You’ll assist residents with activities of daily living, support mobility and fall prevention, and help coordinate care with the broader clinical team to ensure smooth care transitions. You’ll participate in care planning, contribute to wound care when needed, manage infection control practices, and uphold safety standards in every interaction. You’ll leverage your clinical judgment to identify potential issues early and communicate clearly with supervising RNs and physicians.
