Role Specifics and Benefits:
- Key responsibilities:
- Conduct thorough in-home patient assessments and develop individualized, evidence-based care plans.
- Manage chronic conditions, coordinate medications, and monitor treatment responses to optimize outcomes.
- Perform wound care, IV therapy, and acute/complex nursing interventions with a focus on safety and infection prevention.
- Educate patients and families on disease management, medication adherence, safety at home, fall prevention, and rehabilitation exercises.
- Coordinate with physicians, therapists, social workers, and community resources to ensure seamless transitions and comprehensive support.
- Document care activities accurately in the electronic health record and ensure compliance with regulatory standards.
- Facilitate transitions from hospital to home, organize durable medical equipment, and advocate for patient access to necessary services.
- Utilize telehealth and remote monitoring tools to support follow-up visits and triage as appropriate.
- Travel between patient homes as needed, with robust support from the company’s travel services to ensure smooth assignments.

- Guaranteed Hours: 40.0 per week.
