• Unit: Long-Term Care and Rehab Nursing
• Patient Population:
  • Sub-acute patients needing rehabilitation
  • Long-stay residents requiring skilled nursing care
• Duration: 16 weeks

• Deliver exceptional nursing care to rehab and long-term care patients in a supportive and collaborative environment.
• Work with interdisciplinary teams to develop and execute patient-centered care plans that promote recovery and long-term health.
• Document care accurately in EPIC EMR (recent experience required).

📋 Requirements

• Certifications:
  • BLS
  • NIH Stroke Scale (must be completed prior to start)
• Experience:
  • Prior experience in rehab and long-term care nursing required
  • Minimum 2 years of experience in a large teaching facility
  • Travel nursing experience preferred
• Licensure:
  • Vermont RN license or proof of application required at the time of offer
• Documentation:
  • Complete employment history, including facility details, bed count, EMR used, and dates
