In this Long-Term Care LPN role, you’ll be at the heart of resident stability and quality of life. Key responsibilities include direct patient care, medication administration, monitoring vitals and chronic condition management, wound care, pain control, and assistance with activities of daily living. You’ll participate in care planning discussions, document thoroughly in electronic medical records, and communicate changes in patient status to the supervising RN and family members with clarity and empathy.
