Role Specifics and Benefits
As a Long-Term Care LPN, you will play a pivotal role in daily resident care and care planning. Core responsibilities include administering medications under physician orders, monitoring vital signs, assisting with activities of daily living, wound care, and feeding, and documenting observations accurately to guide clinical decisions. You’ll collaborate closely with RNs, CNAs, therapists, and family members to implement individualized care plans, prevent complications such as pressure injuries or infections, and respond promptly to changes in residents’ conditions. Your work will also encompass coordination during admissions and discharges, ensuring continuity of care and a respectful, resident-centered environment.
