Role Specifics and Benefits: In this Long-Term Care LPN role, you’ll provide direct, hands-on nursing care to residents, guided by individualized care plans and interdisciplinary teamwork. Key responsibilities include administering medications, monitoring health status, identifying changes in condition, coordinating with physicians and therapists, documenting accurately, and contributing to the development and adjustment of care plans.
