🔹 Position Overview:

The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching. This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities.

🏥 Key Responsibilities:

✔ Provide direct patient care under the supervision of RNs or physicians.
✔ Conduct health coaching sessions to promote wellness and chronic condition management.
✔ Perform accurate medical documentation in compliance with healthcare regulations.
✔ Assist in outpatient settings, LTC, and hospital medicine environments.
✔ Administer medications and treatments as prescribed.
✔ Monitor patients’ vital signs and report any changes.
✔ Support patients undergoing dialysis and educate them about the process.
✔ Utilize medical terminology to communicate effectively with healthcare teams.
✔ Conduct diagnostic evaluations to assess patients’ health status.
✔ Ensure infection control protocols and maintain a safe patient environment.

🎓 Requirements:

📌 Active LPN License in the state of practice (VT, NH, or compact license).
📌 Minimum 2 years of Long-Term Care (LTC) experience.
📌 Must be able to adapt quickly and work with minimal orientation.
📌 Experience working in memory care settings preferred.
📌 Excellent communication skills – verbal and written.
📌 Ability to work collaboratively while maintaining patient confidentiality.
📌 Flexibility to adapt to varied healthcare environments and schedules.
