In your daily responsibilities, you’ll conduct comprehensive home health assessments, develop and implement patient-centered care plans, and reevaluate progress in collaboration with physicians, therapists, and social workers. You will manage wound care, ostomy or catheter care, injections, infusion therapy when appropriate, administer medications as prescribed, monitor vital signs, assess safety risks, and provide thorough patient and caregiver education on disease management, nutrition, exercise, medication safety, and emergency planning. You’ll coordinate seamlessly with the broader care team to ensure smooth transitions, prevent hospital readmissions, and align services with patient goals. The role includes professional growth opportunities: pursuing specialty certifications, participating in evidence-based practice projects, and gaining exposure to a range of chronic conditions common in home health. We offer competitive compensation, with weekly pay between $2,297 and $2,402, guaranteed 40 hours per week, and a start date of February 23, 2026. The package also includes a signing bonus for eligible candidates, housing assistance or stipends to help ease relocation or temporary housing, and the possibility of contract extensions based on performance and organizational needs. We provide robust support, including 24/7 assistance while traveling with the company, dedicated travel coordinators, and on-call clinical leadership to troubleshoot patient care challenges, equipment needs, documentation, and safety concerns. You won’t navigate complex cases alone; you’ll have immediate access to a nationwide network of clinicians and a responsive support team that helps you stay focused on high-quality patient care, wherever your assignments take you.
