Role specifics and benefits
Key responsibilities:
- Perform comprehensive PT evaluations, establish individualized treatment plans, and set measurable functional goals aligned with diagnoses and patient priorities.
- Deliver evidence-based interventions, including therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training, and modality use to reduce pain and restore function.
- Monitor progress, reassess outcomes, and adjust plans while maintaining thorough, compliant documentation in the electronic medical record.
- Educate patients and families on home exercise programs, activity modification, injury prevention, and self-management strategies to sustain gains after discharge.
- Collaborate closely with multidisciplinary teams to ensure seamless, patient-centered care and safe transitions across settings.
- Uphold safety, infection control, and regulatory standards; participate in quality improvement and peer-learning activities.
