In this role, you’ll perform comprehensive in-home assessments, develop evidence-based care plans, manage medications, oversee wound care and IV therapies where applicable, monitor symptoms, and execute safety-focused interventions in diverse home environments. You’ll coordinate with interdisciplinary teammates, including therapists and social workers, to address medical and social needs, ensuring smooth transitions between care settings. Your educational guidance to patients and families will empower adherence, promote safety, and enhance quality of life.
