As an LPN in Long-Term Care, you’ll be at the frontline of daily resident care: monitoring vital signs, administering medications, assisting with activities of daily living, wound care, mobility support, and fall prevention. You’ll collaborate with RNs, physicians, therapists, social workers, and families to implement and refine care plans, document in electronic health records, and participate in rounds that shape outcomes. Your role also includes resident and family education, recognizing changes early, and coordinating transitions.
