As a Travel RN Case Manager, you’ll own key responsibilities that advance patient care and strengthen care coordination. Conduct comprehensive assessments, identify risk factors, facilitate timely discharge planning, and arrange post-acute services, home health, durable medical equipment, and medication management. Lead utilization reviews and care transitions, collaborating with multidisciplinary teams to remove barriers to optimal outcomes. You’ll work with patients and families to address social determinants of health, coordinate resources, and ensure continuity of care as needs evolve. This role offers meaningful professional growth within case management, enabling you to deepen expertise in chronic disease management, care coordination, and health system operations. The position is designed for a multi-week engagement, beginning around 04/07/2026, with competitive weekly pay ranging from $2,238 to $2,390. A completion bonus is available, along with housing assistance or stipends and opportunities for extension based on performance and program needs.
