Key responsibilities include conducting thorough bedside assessments, administering medications and treatments, wound and IV therapy management, and advanced clinical interventions as prescribed. You’ll develop and implement personalized care plans, monitor symptoms, adjust regimens in collaboration with physicians, and educate patients and families on disease processes, treatment goals, medication safety, fall prevention, and chronic disease management. You’ll coordinate with social workers, therapists, and primary care teams to ensure smooth transitions of care, arrange durable medical equipment, and facilitate timely referrals. Documentation is meticulous, compliant, and timely, ensuring continuity of care and accurate communication across teams. There’s room for professional growth through advanced certifications in home health care, wound care, or palliative approaches, and a clear pathway to leadership roles within the field as you accrue experience.
