Role specifics and benefits: As an LPN in Long-Term Care, you’ll provide essential nursing care under the supervision of registered nurses and physicians. Your responsibilities will include administering medications and treatments according to physician orders and care plans; monitoring vital signs and responding promptly to changes in residents’ conditions; assisting with activities of daily living and promoting residents’ independence; delivering wound care and skin integrity management; performing routine assessments, documenting progress, and communicating updates to the care team; collaborating with an interdisciplinary group—including RNs, physicians, social workers, therapists, and dietary and activity staff—to implement individualized care plans; supporting residents’ families with education and care coordination; and contributing to a safe, respectful, and dignified care environment.
