In this role, you’ll assume essential responsibilities that honor the LTC setting: comprehensive assessments to chart evolving needs, medication administration and reconciliation, wound care, catheter and incontinence management, fall prevention, and timely response to changes in condition. You’ll develop and routinely update individualized care plans, coordinate multidisciplinary rounds, document meticulously in electronic health records, and maintain patient safety through evidence-based practices and infection control standards. You’ll monitor residents’ functional status, support rehabilitation goals, and empower families with clear, compassionate communication about care plans and transitions.
