In this role, you’ll carry out essential responsibilities with independence and accountability, including assessing residents’ conditions, administering medications under supervision, monitoring vital signs, and documenting changes in care plans with accuracy and timeliness. You’ll collaborate with registered nurses, physicians, therapists, and social services to coordinate activities of daily living, nutrition support, and restorative therapies, ensuring continuity of care across shifts. You’ll help manage chronic conditions typical in long-term care, assist with ADLs, wound care, infection control, and fall prevention, and support families by communicating care goals and progress.
