Step into a pivotal role as a Registered Nurse specializing in Case Management, where your expertise will guide patients through complex care journeys and help families navigate resources with confidence. In the striking landscapes of Arizona, including Tuba City, you will weave evidence-based care coordination with compassionate advocacy, ensuring individuals receive timely, appropriate services that improve outcomes and quality of life. This assignment begins on 04/06/2026 and offers a structured schedule designed to support professional growth while honoring your commitment to patient-centered care. Join a program that values your clinical judgment, your ability to build trust, and your drive to close gaps in care for diverse populations. And beyond the clinic, you’ll have the chance to explore Arizona’s iconic beauty—from the red rock formations of Sedona to the vast canyons near the Grand Canyon and the crisp, star-filled nights that frame the state’s expansive horizons. This is your chance to make a lasting impact while experiencing the unique culture and geography of the Southwest.
Location Benefits: Tuba City sits at the crossroads of history, culture, and majestic scenery. Work in a community that blends Navajo traditions with modern healthcare, offering opportunities to develop relationships with patients and families in meaningful ways. You’ll collaborate with physicians, social workers, therapists, and community health workers in a setting where caseloads require thoughtful prioritization and creative problem-solving. And if you’re excited by the prospect of refining your practice across different environments, our program also presents the option to travel to varied locations across the U.S., expanding your professional horizons while maintaining the stability of a guaranteed 36 hours per week. The surrounding landscapes invite reflection, while the daily patient interactions provide tangible avenues to impact lives, making every shift both purposeful and rewarding.
Role Specifics and Benefits: As a Case Management RN, you will assess patient needs, develop comprehensive care plans, coordinate services across inpatient and outpatient settings, and facilitate safe transitions from hospital to home or to next level of care. Your responsibilities include conducting risk assessments, coordinating with social determinants of health resources, managing referrals, tracking outcomes, and ensuring adherence to evidence-based protocols. You’ll document progress, communicate with payers, support discharge planning, and educate patients and families to empower self-management. This role is designed to foster professional growth: you’ll gain depth in complex case management, palliative and chronic disease coordination, and care transitions, with access to certification prep, mentorship, and potential leadership opportunities within the program. We offer a competitive compensation package, including a sign-on bonus, housing assistance, and the possibility of contract extensions to longer-term assignments as you demonstrate your impact. In addition to a guaranteed weekly pay of $2,364-$2,423, you’ll receive comprehensive benefits that reflect the value we place on your expertise. You’ll have 24/7 support while traveling with the company, ensuring you’re never alone when you need guidance, resources, or immediate assistance. This structure is designed to minimize downtime between assignments, keep your clinical skills sharp, and provide ongoing opportunities to expand your scope of practice across diverse patient populations and settings. You’ll also benefit from a streamlined onboarding process, access to continuous education resources, and a supportive team that respects your autonomy while providing reliable backup when complex cases arise. The program includes housing assistance, flexible scheduling within the guaranteed 36 hours per week, and clear opportunities to extend your contract if you choose to remain with the organization or pursue additional assignments across the United States.
