In this role, you’ll own a broad spectrum of responsibilities that foster professional growth within the LTC specialty. Core duties include comprehensive patient assessments, medication administration, wound and skin care, infection prevention, pain management, and fall-risk reduction, all documented accurately in the electronic medical record. You’ll collaborate with physicians, therapists, social workers, dietitians, and case managers to implement, monitor, and update individualized care plans. You’ll monitor vital signs, manage chronic conditions common in long-term care, and respond rapidly to changes in residents’ conditions. Your communication will extend to residents and families, ensuring clarity, compassion, and expectation alignment.
