In this role, you’ll deliver essential long‑term care nursing to residents who rely on skilled assessment, compassionate supervision, and precise care planning. Your responsibilities will include performing comprehensive assessments, administering medications and treatments, wound care, and catheter/venous access management as appropriate, monitoring vital signs, documenting progress notes, and contributing to individualized care plans in collaboration with RNs, physicians, social workers, and therapy teams. You’ll support daily living activities, implement mobility and rehabilitation goals, educate residents and families about health maintenance, and participate in care transitions to preserve continuity of care.
