In this role, you’ll provide comprehensive direct patient care to residents in long-term care settings, integrating clinical expertise with empathy and clear communication. Your key responsibilities include accurate administration of medications and treatments, skilled assessment of residents’ conditions, monitoring vital signs, managing chronic diseases, wound care, and rehabilitation support to promote mobility and independence. You’ll develop and implement individualized care plans in collaboration with physicians, therapists, social workers, and family members, ensuring that each resident’s goals and preferences remain central. You’ll engage in fall prevention initiatives, infection control practices, and safe mobility programs while documenting care with precision in electronic health records. You’ll also mentor and orient new staff, model best practices in patient safety, and advocate for residents’ comfort, dignity, and rights.
