- Key responsibilities: Provide high-quality, evidence-based direct patient care to LTC residents, including comprehensive assessments, monitoring and management of chronic conditions, wound and IV therapy, medication administration, and treatment plan execution. Deliver restorative and rehabilitation activities, assist with activities of daily living, and implement fall prevention and safety protocols. Collaborate with physicians, therapists, dietitians, social workers, and families to develop, implement, and update individualized care plans. Ensure accurate documentation, timely communication, and compliance with regulatory standards. Educate residents and families on care plans, medications, and disease management, and coordinate transitions of care and discharge planning with the broader care team.
