As a Long-Term Care LPN, you’ll be the steady advocate for residents facing chronic conditions, mobility challenges, and age-related changes. Responsibilities include administering medications and treatments within scope, performing comprehensive assessments, contributing to and updating individualized care plans, monitoring vital signs, coordinating with RNs and multidisciplinary teams, documenting accurately in electronic health records, managing wound care and infection prevention, assisting with activities of daily living, and providing emotional support to residents and families.
